Create Your AFL Claims Report
Turn your condition files into one organized report. Select the PDF button, then choose Save as PDF in your device's print window. The report is designed to read, print, and take to an accredited representative.
The JSON file is only a technical recovery copy; it is not the readable claims report.
Start Your First Condition File

Finish a final accuracy check, assemble the report with its supporting records, and have the complete packet reviewed before filing.

Read every condition card once more. Correct anything inaccurate and make sure the report clearly separates what you know from what still needs evidence.
- Condition name and diagnosis status are accurate
- Service event, onset, or secondary connection is explained
- Symptoms and real-life impact are described plainly
- Missing records or opinions are identified

Print and Add Your Records
Print the AFL report and place it at the front. Then organize the source documents that support each condition behind it.
- Service treatment and personnel records
- VA and civilian medical records, tests, and DBQs
- Orders, LODs, exposure records, and official reports
- Personal, buddy, caregiver, or witness statements
Remember: The AFL report helps organize the claim. It does not replace medical records, service records, DBQs, nexus evidence, or statements.
Get a Free Accredited Review
Take the AFL report and the supporting records to a VA-accredited representative. Ask them to review the claim before anything is submitted.
- Does each condition have the evidence it needs?
- Is direct, secondary, aggravation, or presumptive service connection supported?
- Are the correct forms and effective-date issues addressed?
- Should anything be developed further before filing?
VA accreditation matters: Verify anyone helping prepare or file the claim through VA’s official accreditation search.
Strengthen and Understand the Packet
Use these tools to locate missing evidence and understand the rating picture. They are preparation aids—not a VA decision or guarantee.
Protect your records. Keep original documents secure and provide copies whenever possible. Do not email Social Security numbers, VA file numbers, or unredacted medical records unless the recipient has provided a verified secure method.
